Practice Charter
We aim to provide high quality, personal care to our patients through, positive,
supportive teamwork.
The care of your health is a partnership between yourself and the Primary Health
Care Team. The success of that partnership depends on an understanding of each other’s
need and co-operation between us.
Our Commitment to You
- You will be greeted courteously.
- We will keep confidential records to which, (subject to the limitations of the
law), only you and those involved in your care have access.
- We will involve you in your care, respect your confidentiality and when necessary,
refer you for specialist treatment and investigation.
- You have the right to see your medical records subject to the limitations of the
law.
- You will be seen the same day if your problem is urgent.
- You will be seen by a named doctor whenever possible.
- You will be informed if there will be a delay of more than 30 minutes for your appointment.
- You will be given the result of any test or investigation as timeously as possible.
- Your repeat prescription will be ready for collection or at the chemist of your choice
within 48 hours of your request.
- Your suggestions or comments about the services offered will be considered sympathetically
and any complaint dealt with promptly.
Your commitment to Us
- Please treat all staff with the same respect—we are all just doing our job.
- Do not ask for information about anyone other than yourself.
- Tell us of any change of name, address or telephone number, so that our records are
accurate.
- Only request an urgent appointment if it is appropriate.
- Home visits should only be requested if you are too ill to attend the surgery. If
in doubt, discuss it with out duty doctor please.
- Please remember to advise us if you are unable to attend as appointments are valuable
and needed by others.
- Please be punctual, but be prepared to wait if your own consultation is delayed by
an unexpected emergency or someone needing more help than can be provided in a standard
length of consultation.
- Help us to help you by telling us a clearly as you can how you are feeling, it may
help to write it down. If you do not understand what your doctor / nurse asks or
tells you, please say so.
- Please allow sufficient time for your consultants letter or the results of any tests
to reach us.
- Use the tear off slip to request your repeat prescription whenever possible. Please
attend for review , when asked, before your next prescription is due.
- Do let us know whenever you feel we have not met our responsibility to you.
- We would, of course, be pleased to hear when you feel praise is due as well.
